bonhambenefits is a unique program that puts you, the patient, back in control of your dental health care without straining the budget.  For a small monthly fee, you and your family can rest knowing they will always have the preventative care they need without coming out of pocket.  Plus, bonhambenefits makes it possible for patients without dental insurance to afford the best dental care possible while offering a substantial discount on treatment most insurance companies would never cover including cosmetic procedures like Whitening, Lumineers and Invisalign.

Procedures included in the program:

  • Preventative Procedures covered with the Bonham Benefits program include:  (2) Professional Cleanings or Periodontal Maintenance Treatment per year (ADA #1110, #1120, #4910).
  • The following X-Rays are included: (1) FMX or Bitewings (1) Panoramic included when necessary (ADA #210, #274, #330).
  • The following Exams are included: (1) Complete Exam or Periodic Exams (# 0150, #0120).

Billing

  • Bonham Benefits occur on an annual cycle and automatically renew unless the patient notifies beforehand.
  • Participants may cancel at anytime but may be charged a $60 cancellation fee.
  • Patients pay the first month’s installment on the day of their first appointment or on the day they decide to take advantage of the benefits.
  • A valid credit card or debit is required to be part of the program.  The respective monthly fee will be charged to the credit card on file on the 1st day of every month following initial payment.   Checks or cash will not be accepted for the recurring monthly fee but may be used for the first payment.

Restrictions

  • A valid email address is required for Bonham Benefits communication.
  • Dental insurance cannot be utilized in conjunction with Bonham Benefits
  • Bonham Benefits offered by Bonham Dental Arts is not a health insurance policy.
  • Bonham Benefits offered by Bonham Dental Arts provides discounts solely at Bonham Dental Arts alone for medical services and cannot be used at any other dental office.
  • The Bonham Benefits member is obligated to pay for all health care services not included in the plan but will receive a predetermined discount for all services performed.
  • Dental insurance cannot be utilized in conjunction with this program.
  • This offer applies to New Patients and Current Patients alike.  However,  treatment performed prior to participation will not be eligible for reimbursement or discount.
  • May not be combined with any other offer or discount.
  • The family plan includes 2 adults (over 18 years old) and up to 4 children (under 18 years old).
  • The patient and any other person responsible for payment has the right to refuse to pay, cancel payment, or to be reimbursed for any other service, examination, or treatment that is performed as a result of and within 72 hours of responding to the advertisement for the free, or reduced fee service, or treatment.
